There are a variety of advance care directive forms available to record your wishes and select your health care agent. Advance directive laws differ from state to state. If you move, you should check with an attorney or healthcare provider to ensure that your advance directive is valid.

NC Advance Health Care Directive Registry
The North Carolina Department of the Secretary of State has established a registry where one may file their Advance Health Care Directives. They may include the following:
- A health care power of attorney
- A declaration of a desire for a natural death
- An advance instruction for mental health treatment
- A declaration of an anatomical gift

Advance Care Planning - How to Talk About Your Health Care Choices
This is a free, downloadable brochure and FAQs abut advance care planning for South Carolinians. It is a joint effort of TCC, the SC Medical Associtaion, the SC Hospital Association and the SC Bar. This is useful for consumers and health care professionals.
Advance Care Planning includes:
