Hospice of the Carolina Foothills Founded in 1981 as Hospice of Polk County, our Medicare-certified, not-for-profit, free-standing agency has continuously, since 1981, provided care for individuals, and their families, who live with serious and terminal illness, grief and loss. Our staff of competent, compassionate professionals provide expert care and support, meeting the medical, emotional, and spiritual needs of individuals and families affected by chronic or terminal illness, death and grief. Physician (Full or Part Time):
The physician will provide palliative care services for individualized symptom management and quality of life issues in a variety of settings including hospice home visits, skilled nursing facilities, our 12-bed inpatient hospice facility, and will work closely with the Medical Director. Must be eligible to apply for or possess a current and unrestricted medical license in North Carolina and South Carolina, minimum of three years experience in medical practice, hospice and palliative medicine experience and board certification in hospice and palliative medicine preferred. Nurse Practitioner (Full or Part Time)
The Nurse Practitioner will provide palliative care services for individualized symptom management and quality of life issues in our 12-bed inpatient hospice facility, and will work closely with the Medical Director. Must be certified as family, geriatric, or adult nurse practitioner. Prefer 2 years experience as nurse practitioner, preferably in palliative, geriatrics, or end of life care. EOE.
Hospice Liaison (Full-Time)
The Hospice Liaison connects the services of HoCF with the patient/family needs in physician practices, hospitals, and nursing facilities. Also assists in community education and public events. Minimum of two-year associate degree-bachelor's degree preferred-in Sales, Marketing, Communication; or clinical healthcare degree. Minimum of four years experience in sales or healthcare delivery required; hospice experience preferred. EOE. Registered Nurse
Medi Home Hospice, a division of Medical Services of America, Inc. currently seeks an experienced Hospice Registered Nurse (PRN) to join our team in Rock Hill, SC.
As a member of the multidisciplinary team, the RN works under the general direction of the Director of PCC by providing support to the patient/family to provide care and to maximize the comfort and health of patients and families. The RN is responsible for:
- Planning and providing care for patients
- Adherence to policies and procedures
- Assisting in identifying patient/family needs and communicating with physician
- Assisting with providing supportive care in accordance with the attending physician's orders

Family Support Coordinator - Hospice
Medi Home Hospice, a division of Medical Services of America, Inc. currently seeks an experienced Family Support Coordinator to join our team in Rock Hill, SC.
As a member of the multidisciplinary team, the Family Support Coordinator works under the general direction of the Director of Director of Family Support. The Family Support Coordinator is responsible for:
- Assisting and counseling clients and families with financial, social and emotional concerns in order to maximize the comfort and health of patients and families.
- Assesses patient/family social and emotional factors in order to estimate their capacity and potential to cope with their terminal illness and death
- Assists patient, family and staff in utilizing community resources
- Assesses patient/family ability to meet financial obligations and offer appropriate resources and activate as indicated
- Assists in discharge planning from hospice service when care is no longer appropriate
- Coordinates with patient, family and multidisciplinary team members to develop and implement the plan of care
- Evaluate patient referrals and assist with admissions as needed
- Provides bereavement follow-up services as defined by procedures
- Accepts other assignments as may be appropriate
